Large Group - over 100 lives:
Claims history for the past two years including shock loss information of any claims totaling more that $10,000.
Groups between 50 and 100 lives:
A risk assessment questionnaire will need to be completed by the employer. This questionnaire includes health questions about the group. (Will provide upon request)
Groups under 50 lives:
Each eligible employee will be required to complete an application that includes health questions. These applications will then be reviewed by the insurance carrier to provide the group with "firm" rates.
What is the participation requirement?
General participation requirements for a group to be eligible for insurance are as follows:
Minimum participation is 75% excluding waivers for those employees covered by other group insurance, Medicare or Medicaid. The minimum participation for all eligible employees is 50%.
What do I need to contribute to the plan?
Employer contributions vary slightly from carrier to carrier. The general rule is the employer must contribute at least 50% of the single premium.
